UNLV Optional Insurance Programs

The following programs are offered as optional programs for UNLV employees. Most are available through payroll deduction and some through salary reduction.

Cancer Care Insurance

American Fidelity Insurance 1-800-654-8489 or 1-702-433-5333

Offers two Cancer Insurance Plans employees may choose from: Plan I, the Cancer Expense Protector, and Plan II, the Cancer Protection Plus. Both plan will provide coverage for the actual charges incurred by a covered person for the treatment of cancer, leukemia, or Hodgkins disease, subject to certain maximum amounts. The plan is available to employees under the age of 65 who have not previously been diagnosed as having cancer. Employee only and family plans are available. A two year waiting period for coverage is required if cancer is diagnosed within 30 days after the date of issue. The effective date is the first of the month following the payroll deduction.

Personal Accident

CNA Insurance Company 1-800-303-9744

Offers coverage for accidental death or dismemberment, permanent and total disability, or paralysis. Employees under the age 70 may enrll in this plan at any time without having to provide evidence of insurability. Employee Only and Family Plans are available. The effective date is the first of the month following the payroll deduction

Personal Recovery

AFLAC 702-597-2788

Offers financial assistance if you are faced with expensive medical treatments and loss of income for certain specified health events. These specified evetns include: heart attack, stroke, coronary artery bypass, surgery, coma, paralysis, major third degree burns, end-stage renal failure, and major human organ transplant. AFLAC Pays the plan benefits directly to you so that you can determine the best use for this additional income.

Your age and the level of coverage you select determine the cost of this plan. Premiums can be withheld on a pre-tax basis.

Pre-Paid Legal Services

Metlaw 1-800-821-6400

MetLaw provides you, your spouse or domestic partner, and dependents with full covered legal services from experienced attorneys at a low monthly group rate ($15.75/month), paid through payroll deductions. Short Term Disability

Replaces a portion of your monthly income if you become totally disabled on a short term basis. Benefits normally begin on the 15th day of disbaility and pays up to 26 weeks or age 70, whichever occurs first. If approved, the effective date is the first of the month following the payroll deduction. UNLV offers this plan through two vendors:

Supplemental Term Life Insurance

UNLV offers supplemental term life insurance for employees through two vendors - Western Insurance Specialties and Standard Insurance. Active employees may apply for life insurance up to a maximum of $500,000.00.

Western Insurance Specialties 1-800-342-0707

Offers a guarantee issue coverage of up to $150,000.00 for the employee and $30,000.00 for their spouse/domestic partner providing that the employee applies for the guarantee issue within 60 days of their date of hire. After that, employees will have to submit evidence of insurability for any amount of life insurance requested. Dependent childen may be insured up to a maximum of $20,000.00 as long as either the employee or the spouse is also covered.

Standard Insurance 1-888-288-1270

Offers a guarantee issue coverage of up to $100,000.00 for the employee and $20,000.00 for their spouse providing that the employee applies for the guarante issue within 60 days of becoming eligible for the State Health Plan. After that, employees will have to submit evidence of insurability for any amount of life insurance requested. Employees must be a participant in the State's Health Plan in order to qualify for this benefit.

Voluntary Indemnity Plan

Under this program, AFLAC will pay benefits for daily hospital confinement, for illness, or for accident-related events. Additional benefits are payable when you have a surgical procedure. Ambulance benefits are also payable. Cash benefits are paid directly to you.

Your age and the level of coverage you select determine the premiums. Premiums may be withheld from you pay on a pre-tax basis.

Long Term Care

This program provides you with assistance if you have a chronic condition that leaves you unable to care for yourself over an extended period of time. Long-term care may be received in your home, in an adult day care center, in an assisted living facility, a nursing home or in other locations. This policy generally provides benefits for expenses not covered by Medicare or your health insurance plan. Your age and the level of coverage you select determine your premiums.
