VISION BENEFITS

Vision Benefits are included as part of your health insurance package. Benefits differ depending on whether you are on the PPO Plan or the HMO Plan.

Under the PPO Plan, participants can go to any vision provider and submit a claim directly to Benefit Planners for reimbursement. The Plan covers one vision exam every 12 months, or a rolling one year period, paid at 80% of Usual and Customary charges. There is also a $125 hardward allowance for contacts, lenses, and frames every 24 months, or a rolling 2 year period. Under the HMO Plan, coverage is only available if the participant goes to an EyeMed Provider. The HMO plan covers one vision exam per year and the participant pays a $10 co-pay. Additional information regarding coverage for Lenses, Frames, and Contact lenses is available at HPN website.
