Employment documents are the documents used to communicate additions, modifications, or deletions regarding an employee's status and/or

Academic/Administrative Faculty Policy on Compensation in excess of Base Salary
Signatures that should be on the Request for Extra-Contractual/Supplementary Compensation form are as follows:
(1) Employee Signature
(2) Principle Investigator/Account Manager for department submitting Employment Document
(3) Chair/Supervisor of the Employee's Primary Appointment (This may be different than the department for which the ECC is being requested)
(4) Dean/Director of the Employee's Primary Appointment (This may be different than the dean/director for which the ECC is being requested)
(5) Office of Sponsored Programs (Applicable only if funded from Grant Money)
(6) Vice President of the Employee's Primary Appointment (This may be different than the vice president for which the ECC is being requested)
