Acknowledge Applications

The department will acknowledge applications by sending an initial response letter, an EEO Survey Form, and Voluntary Self-Identification Form with a Business Reply Envelope to all applicants. Human Resources will supply the department with the EEO Survey Form, Voluntary Self-Identification Form and as many envelopes as necessary. It is the responsibility of the department to make copies of the forms provided by Human Resources. Each recruitment will be assigned a Search Number which will appear on the forms, along with the Title and Department. For those departments with recruitments occurring simultaneously, it is important to ensure the correct forms are sent to each applicant. If additional items are required to complete an applicant’s file, this mailing would provide an be the appropriate opportunity to request those documents.

Note: It is recommended that the search committee chair keep a file/spreadsheet that lists the candidate’s name and date each response letter, EEO Survey Form, Voluntary Self-Identification Form and regret letter was sent to each applicant. It is the responsibility of the chair to ensure that all applicants receive a response letter which acknowledges receipt of their application materials by the department, and that each applicant receives an EEO Survey Form and Voluntary Self-Identification Form so that they have the opportunity to voluntarily submit EEO information to Human Resources and the Office of Diversity Initiatives for statistical purposes. Human Resources reserves the right to review all files or other information collection methods used at anytime during the search.
